Health Insurance Card/Information
Photo ID
A list of all medications you are taking or bring all your current medications for review
A copy of your XRAYS / CT scans
Copy of records from doctors related to your visit with us
Completed NEW PATIENT FORM
Things you should bring to follow up visits:
- A list of all medications you are taking or bring all your current medications for review
- Any diagnostic studies (MRI/CT Scan, Xray, etc...) or medical records
Appointments
Your first visit to our office is a consultation and will last from 45 to 60 minutes. During the consultation your doctor will discuss your condition and determine a treatment plan if one is indicated. Due to space constraints, only one other person will be allowed in the exam room with the patient during the patient/provider examination.
Follow up visits are scheduled at the end of each visit.
Patients who are late for their appointment and/or check-in time may need to be rescheduled.
Prescription Policy
It is our office policy to refill prescriptions only during clinic hours. This means that refills will not be given after clinic hours and/or the weekend. We also will not refill early prescription. Medications are to be taken ONLY AS PRESCRIBED. Opiate or scheduled medications WILL NOT BE REFILLED without an appointment.
